Affordable Mineral Makeup Independent Representative Agreement
Affordable Mineral Makeup and Teen Mineral Makeup ("AMM") and the individual submitting this Independent Representative Agreement ("Agreement") ("Representative" or "I" or "my") enter into this Agreement for the purpose of representing and selling the products and services made available by AMM to Representative under the terms and conditions of this Agreement.
Representative agrees that the information provided on the application is complete and true and that Representative is a legal U.S. Citizen or resident and over the age of 13.
Representative acknowledges that she/he is not and will not be an employee of AMM, and that she/he is provided no guarantee of income, wages or other benefits.
Any income derived through Representative's association with AMM will be based solely upon Representative's own efforts to sell and market the AMM products to customers. Representative agrees to keep track of all earnings and be responsible for payment of all applicable State and Federal taxes as a self employed person. Representative agrees to comply with all Federal, State and local rules and regulations pertaining to taxes.
Representative agrees that she/he will make the necessary arrangements to obtain the information for tax purposes and business purposes through Representative's own state of residence. Representative agrees to research any and all personal and business related tax consequences that may pertain to independent representation of the products of AMM and that it is not the obligation of AMM.
Representative agrees to sell AMM products by personal sales, online sales through Representative's page or pages on the AMM websites, home parties or other such venues as may be approved by AMM. Representative may sell products of AMM in conjunction with other products from other companies, but may not sell them under that company’s name.
Representative agrees to receive a 15% discount on each order Representative places with AMM, and a 15% commission on orders placed by verified Representative customers for AMM products.
Representative will receive 30% off one initial order to ‘build their personal representative kit’. No kit is sent with Representative start up.
If AMM discovers Representative has accepted monies for customer orders and not placed orders with AMM in a timely fashion or otherwise acted inappropriately, AMM reserves the right to immediately terminate Representative from the AMM Representative program.
Commissions are paid to Representative once per month on all qualified orders and are paid via PayPal or check (if paid by check there is a $1 service charge). Commissions are paid the first week of the following month. There is a no-return policy on all products.
Representative will have an AMM personal shopping catalog page for Representatives in good standing. Representative must use this link for all leads, clients and customers to ensure that orders processed through the AMM website will be appropriately credited to them. AMM will process all orders and credit card transactions for Representative for orders placed from the Representatives page. Representative will have a link similar to the following example:

AMM has extensive experience in direct sales and will make every reasonable effort to ensure satisfaction with the Representative program and AMM products. Either the Representative or AMM may choose to terminate this agreement at anytime for any reason. AMM reserves the right to modify its policies as necessary.
By completing and Submitting this form, Representative confirms she/he has read and agrees to the terms and conditions set forth in this Agreement with the same force and effect as if you had signed a printed Agreement.
